We provide care overseen by a licensed nurse care manager, who does an in-depth needs, assessment, draws up a plan of recommended care, and matches the individual with a skilled, companionable care provider.

Every Advantage Home Care client has a nurse care manager, in addition to his or her care provider.

She checks in regularly with both individual and caregiver to ensure that the individual’s needs are being met in a safe and caring environment. She communicates with other care providers – such as physical therapists, Medicare nurses, or occupational therapists – to ensure that everything is coordinated, nothing falls through the cracks, everyone has the support and information they need to support each individual’s care regimen.

This additional oversight means that Advantage Home Care can catch needed care plan adjustments that might otherwise slip past a typical care provider’s notice, either because of lack of training or lack of time.
